Request Appointment Screening

The College of Massage Therapists of Ontario (CMTO) and Ministry of Health (MoH) require that all clients scheduling a massage therapy appointment must first review and answer the following questions. For the safety of my clients, my family, and the community, it is essential that these questions be answered honestly. These questions will also be asked to you the day before your appointment as well as the day of your appointment after you arrive at the practice.

Massage therapy treatment must only be provided when the anticipated benefits of treatment outweigh the risks to the client and the RMT. Please read the following questions and if you answer YES to any of the following questions, we ask that you delay making an appointment and contact your healthcare provider immediately.

  1. Have you travelled or been in close contact with someone who has travelled outside of Canada in the past 14 days?

  2. Have you had close contact with anyone with respiratory illness or a confirmed or probable case of COVID-19?

  3. Do you have any of the following new or worsening symptoms or signs not relating to a known cause (i.e. joint pain with arthritis, sneezing with allergies, etc.)?
    • Headache, joint pain, or unexplained exhaustion
    • Cough, runny nose, sore throat, or shortness of breath
    • Difficulty swallowing or new inability to smell/taste
    • Chills, nausea, diarrhea, or abdominal pain

  4. Do you have a fever?
